The following documents must be submitted to the Admissions office to be able to register at Colegio Menor.

• Aplication form with student photo
• Copy of the Ecuadorian identification card or copy of passport for foreign students
• Medical certificates documenting:
  • the student’s general health and all vaccines received
  • the results of an eye exam conducted by an Eye Specialist (Pre kinder and Kinder)
  • the results of an auditory exam by an Ear Specialist (Prekinder and Kinder)
• Copy of report cards from the previous three years
• Discipline report from the previous school (3rd grade and up)
• Recommendation letter from a teacher of the previous school (3rd grade and up)
• Special needs certificates if required
• Presentation letter from a sponsor parent o company
• Diplomas or honor certificates if any 

The following documents must be submitted to the Admissions office upon acceptance to Colegio Menor.

Play Group, Pre-kinder, and Kinder:

• Original birth certificate or a copy of the Ecuadorian identification card.  Foreign students may submit a copy of the passport.
• Medical certificates documenting:
  • the student’s general health and all vaccines received
  • the results of an eye exam conducted by an Eye Specialist (Pre kinder and Kinder)
  • the results of an auditory exam by an Ear Specialist (Prekinder and Kinder)

Grades 1 to 6:

• Original birth certificate or a copy of the Ecuadorian identification card.  Foreign students may submit a copy of the passport.
• Medical certificates documenting the student’s general health and all vaccines received
• Original report card from the most recent academic year.  If the school year has not yet ended, this document may be turned in during the student registration period.
• Students entering 1st and 2nd must present the results of an eye exam conducted by an ophthalmologist and an auditory exam by an ear specialist.

Grades 7 to 12:

• Original birth certificate or a copy of the Ecuadorian identification card.  Foreign students may submit a copy of the passport.
• Medical certificates documenting the student’s general health and all vaccines received

• (*) Original:
  • Certificado de Terminación de Primaria
  • Certificados de Promoción
  • Certificados de Matriculas for 7th, 8th, 9th, 10th and 11th grades, depending on the grades that the student has already completed.

(*) If any of these documents have not yet been legalized with the Ministry of Education, students must submit an original yearend report card for the current academic year.  The required documents must then be turned in to the admissions office as soon as they become available.  Parents are encouraged to deliver these documents personally.  Colegio Menor is not responsible for documents sent to school with students, drivers, or other third parties.
